The Mission of The Evergreens is to provide high quality long-term health care and geriatric
rehabilitation services, and to care for all we serve with dignity and respect.

Since its inception in 1964 this private, not-for-profit healthcare organization has been the choice for area residents seeking short-term rehabilitation or long-term nursing care. Our skilled nursing, intermediate and assisted living levels of care provide a continuum, as care needs change. Both The Evergreens’ facilities are located in the heart of the Piedmont Triad in central North Carolina where our beautiful gardens encourage outdoor activities and leisurely strolls.

The Evergreens Senior Healthcare is committed to protecting the privacy of all residents and the confidentiality of their health care information. The Notice of Privacy Practices explains that The Evergreens may use and disclose health information for treatment, payment, nursing home operations, safety, public health needs, emergencies, and as required by law. Residents have the right to restrict disclosure of health information, amend health information, inspect and copy health information, request an alternative method of communication, and request an accounting of disclosures.
