Most new residents enter a nursing home either from home or from a hospital.
HOME
When coming in from home, we like to invite the prospective resident and/or the responsible party to first visit us and tour our beautiful grounds. This gives us an opportunity to show off our facility. We will then obtain some information about the person to be placed, answer any questions about placement and make arrangements to do a home visit where we can assess the resident for care needs.
There are several forms that are mandated by the state of North Carolina.
- FL-2 form - This is a form that has to be completed by the prospective resident’s physician and must be dated within 30 days of admission. This form provides information about a person’s various diagnoses, functional abilities, medications and any special needs. The appropriate level of care is determined by the physician on this form.
- Pre-Admission Screening and Resident Review (PASARR) form. This is a required procedure for all persons entering a nursing home for the first time. It is an attempt to screen out persons who may be placed more appropriately in a group home or psychiatric facility. This form is then faxed to First Health in Glen Allen, VA where it will be reviewed. A number will be assigned to that person to show that they have had this screening. Persons with either mental illness or mental retardation may need a further screening to determine if nursing home placement is appropriate.
These forms are required for admission to this or any other facility. They may be obtained from the County Department of Social Services, physician’s office or from The Evergreens Admission Office.
HOSPITAL
When seeking placement from the hospital, you may want to contact the hospital Social Worker or Case Manager. Let them know that you are interested in placement at The Evergreens. They will assist you with placement and can send us a copy of the FL-2 form and obtain the PASARR number.
It is the policy of The Evergreens to provide adult residents, at the time of admission, with written information regarding their rights to make decisions concerning their medical care, including their right to accept or refuse medical care and to execute advance directives.
